Scott Pilgrim EX is a new, standalone sequel developed by Tribute Games. It follows the team's work on TMNT: Shredder's Revenge and applies that studio's refined beat-'em-up know-how to the Scott Pilgrim license. The story premise: Toronto is fractured by three gangs (the Vegans, the Robots, and the Demons) and reality itself is destabilizing with collapsing locations and time rifts. Scott and his allies must fight through the city to uncover what's happening.

Scott and Ramona are part of the main playable roster. Several of Ramona's former partners are playable too: Roxy Richter, Gideon Graves, Lucas Lee, Matt Patel, and Robot-01 (standing in for the Katayanagi twins). Paid DLC includes Scott's bandmates Stephen Stills, Kim Pine, and Knives Chau.

Each character has distinct special attacks and a few super special moves that consume a resource called guts. Examples cited include Roxy's sword-based ninja moves and Matthew's ability to summon demonic allies. Characters are further differentiated by unique hopping/landing attacks and other basic move variations.

Combat is faster and more fluid than the original Scott Pilgrim game. Enemies have shorter lifebars and are less inclined to turtle, which speeds encounters and reduces repetitive grinding. There are dedicated special attack buttons plus directional inputs for different effects, and super moves activated by combining main and special attack inputs.

Scott Pilgrim EX replaces the original's strictly linear progression with a Kunio/River City–style overworld. A world map shows current objectives, and a "Go!" indicator points to the next goal. The main world largely moves along a straight route through parts of Toronto, but cracks in reality create shortcuts to locations like the beach or Casa Vania (a Casa Loma parody). Warp portals require locating instruments and playing melodies with directional/button commands.

There are twelve main missions and several optional side missions that demand number-of-enemy or time-based objectives. Travel around the map is fast thanks to quick running speeds and Subspace shortcuts.

Tribute Games' refinements tighten combat and character differentiation, but level design leans toward condensation. Areas are typically only a few screens long, so the game can be completed quickly and may feel compressed for players seeking longer stages or exploration.
